dc.description.abstract | Introducción: La EPOC (Enfermedad Pulmonar Obstructiva Crónica) es un grave problema de salud pública mundial. Las hospitalizaciones por exacerbación de la enfermedad son las que generan los mayores costos en salud. La creación de programas de manejo integral podría reducir el uso de recursos sanitarios. Métodos: Estudio de cohorte única, con un diseño de “antes-después” donde cada paciente es su propio control. Se estudiaron pacientes pertenecientes a AIREPOC con al menos una hospitalización de causa respiratoria el año anterior y que no habían participado en programas de rehabilitación pulmonar. La intervención se basó en a) evaluación inicial, b) educación en el autocuidado, c) fácil acceso telefónico, médico y de terapia respiratoria para un seguimiento y atención oportuna de exacerbaciones, y d) plan de cuidado personalizado. La frecuencia de hospitalizaciones, los días de estancia, el índice de disnea y el nivel de calidad de vida fueron comparados al ingreso al programa y al final del primer año en AIREPOC. Resultados: De los 204 pacientes seguidos el primer año de AIREPOC el 70.1% de los pacientes (143) no se hospitalizaron y el total de hospitalizaciones disminuyó un 66.6% (320 a 107). La estancia hospitalaria se redujo en un día pero sin significancia estadística. No se observaron cambios en la disnea medida con la escala del MRCm. La calidad de vida mejoró en todos los dominios del SGRQ, con una disminución en el dominio total de -5.3 u (p<0.001). Conclusión: Un programa de atención integral para la EPOC (AIREPOC), disminuye las hospitalizaciones de causa respiratoria y mejora la calidad de vida durante el primer año de seguimiento. | es_CO |
