Hafnia alvei pneumonia: From bees to human beings

Abstract

Introduction Hafnia alvei is an enterobacteria that is a common inhabitant of the gastrointestinal flora of bees, birds, fish, and mammals. In humans this enterobacteria has been recovered from the oropharynx and the gastrointestinal tract but it has been rarely reported as a pathogen and usually identified as hospital-acquired enterobacteria. Case report We describe a case of a 57-year-old woman, previously healthy, with a 7-day history of cough with brown sputum, sudden onset of chills, subjective fever, malaise, and pleuritic pain in the right hemithorax. A diagnosis of community-acquired pneumonia (CAP) was suspected and empiric antibiotic treatment was started. However, the patient showed no response and developed hemoptysis. A diagnosis of CAP by Hafnia alvei was confirmed with bronchoalveolar lavage and the patient was treated with i.v. cefepime 2 g TID with a good response. Conclusions We presented a case of community-acquired pneumonia by Hafnia alvei in a previously healthy patient that, as far as our knowledge reaches, is the third reported case of CAP secondary to this pathogen. © GERMS 2021.

Introducción Hafnia alvei es una enterobacteria que es un habitante común de la flora gastrointestinal de abejas, aves, peces y mamíferos. En humanos, esta enterobacteria se ha recuperado de la orofaringe y el tracto gastrointestinal, pero rara vez se ha informado como patógeno y generalmente se identifica como enterobacteria adquirida en el hospital. Caso clínico Describimos el caso de una mujer de 57 años, previamente sana, con cuadro de 7 días de evolución de tos con esputo marrón, aparición súbita de escalofríos, fiebre subjetiva, malestar general y dolor pleurítico en hemitórax derecho. Se sospechó el diagnóstico de neumonía adquirida en la comunidad (NAC) y se inició tratamiento antibiótico empírico. Sin embargo, el paciente no mostró respuesta y desarrolló hemoptisis. Se confirmó el diagnóstico de NAC por Hafnia alvei mediante lavado broncoalveolar y el paciente fue tratado con administración i.v. cefepima 2 g tres veces al día con buena respuesta. Conclusiones Presentamos un caso de neumonía adquirida en la comunidad por Hafnia alvei en un paciente previamente sano que, hasta donde alcanza nuestro conocimiento, es el tercer caso reportado de NAC secundaria a este patógeno. © GÉRMENES 2021.
