Multiple sclerosis is a demyelinating chronic inflammatory disease of the central nervous system; it is the main cause of disability of non-traumatic origin around the world in young patients; Although its onset may be heterogeneous, presenting at the beginning any combined or isolated neurological symptom, the most common is that it presents with ophthalmic symptoms, cerebellar syndromes such as gait or balance disturbances, and sensitivity disturbances. This article presents the clinical case of a patient with multiple sclerosis of atypical presentation: dysarthria and expression aphasia without other accompanying symptoms, with a report of a nuclear magnetic resonance in which three supratentorial lesions are evidenced. History of several episodes of visual disturbances, with self-limited blurred vision and vertiginous pictures of short duration. Through this clinical case, the aim is to emphasize the importance of the Primary Care physician to detect early neurological alterations in young patients that lead to suspicion of Multiple Sclerosis and allow an early diagnosis of this disease; On the other hand, the publication of a case like this one with an atypical clinical presentation of Multiple Sclerosis is considered interesting, to contribute with its registration to the increase in knowledge of this disease and the casuistry of future related bibliographic searches. © 2021en
