dc.description.abstract | La histerectomía por vía abdominal es un abordaje de elección para una serie de patologías ginecológicas, aunque implica dolor postoperatorio moderado a severo y mayor estancia hospitalaria, comparada con otros abordajes. El objetivo del presente estudio es describir la intensidad, y la evolución del dolor agudo postoperatorio en pacientes sometidas a Histerectomía Abdominal Total tratadas con bloqueo del cuadrado lumbar en el Hospital Universitario de la Samaritana, en el periodo Julio de 2019 a Julio de 2022. Se realizó un estudio observacional descriptivo de cohorte exploratoria concurrente, en un periodo de tiempo de 3 años. Se incluyeron todas las pacientes programadas para histerectomía abdominal y tratadas con QLB-US en el periodo de tiempo descrito. El bloqueo se realizó siempre guiado por ecografía y todos los anestesiólogos prefirieron el abordaje QL2, como establecen los estándares para la realización del bloqueo. La presencia de dolor severo definida como un puntaje de EVA mayor de 7, se presentó en su mayoría, en el postoperatorio inmediato durante la hora 0, 1 y 2. La presencia de dolor severo disminuyó a partir de la hora 6, 12, y hasta la hora 24. En la presente cohorte de pacientes se describió un efecto analgésico significativo y prolongado en las primeras seis horas postoperatorias, tiempo en el cual, disminuyó el consumo de opioides y sus efectos adversos. | es_CO |
dc.description.abstract | Abdominal hysterectomy is an approach of choice for a series of gynecological pathologies, although it implies moderate to severe postoperative pain and a longer hospital stay, compared to other approaches. The objective of the present article is to describe the intensity and the evolution of acute postoperative pain in patients undergoing Total Abdominal Hysterectomy treated with blockade of the quadratus lumborum at the Hospital Universitario de la Samaritana, in the period from July 2019 to July of 2022. A descriptive observational study of concurrent exploratory cohort was carried out, in a period of 3 years. All patients scheduled for abdominal hysterectomy and treated with QLB-US in the described time period are included. The block was always performed guided by ultrasound and all the anesthesiologists preferred the QL2 approach, as established by the standards for performing the block. The presence of severe pain, defined as a VAS score greater than 7, occurred mostly in the immediate postoperative period during hours 0, 1, and 2. The presence of severe pain began at hours 6, 12, and up to hour 24. In the present cohort of patients, a significant and prolonged analgesic effect was described in the first six postoperative hours, during which time the consumption of opioids and their adverse effects ceased. | en |
