M: To identify the incidence of postoperative delirium due to perioperative pain in hip arthroplasty and to determine the association of perioperative pain with postoperative delirium in hip arthroplasty. MATERIALS AND METHODS: This systematic review was performed on the world literature published up to October 30, 2022. The search, selection and analysis was performed by three independent investigators, with the search terms ((hip arthroplasty) AND (pain)) AND (postoperative delirium) without language or time restriction. In the databases PubMed/Medline 56 articles were identified, Embase 147 articles, Bireme/Lilacs 34 articles and grey literature 32 articles. After the removal of duplicates, the titles and abstracts of 221 studies were read. Forty-five articles were selected for full-text reading. Four studies were definitely selected for the systematic review, which corresponds to 1442 participants, 298 in the delirium group and 1144 in the non-delirium group. Two studies were chosen to conduct a meta-analysis, in total 663 participants were included with 186 patients in the postoperative delirium group. EXPECTED RESULTS: All four (#4) included studies were classified as low risk of bias by the Newcasttle Ottawa scale. The incidence of postoperative delirium in patients with perioperative hip arthroplasty pain was described as 11.4 % to 31.6 %. Two studies were included in the meta-analysis, there were no association between perioperative pain and postoperative delirium in hip arthroplasty (SMD 0.15 95% CI -0.01;0.32, I2 0% p 0.07).en
