dc.description.abstractLa sepsis posee una elevada incidencia y mortalidad, y los modelos de predicción de mortalidad han demostrado ser imprecisos. Un nuevo modelo de predisposición, infección, respuesta y disfunción orgánica (PIRO) plantea abordaje multidimensional y ha evidenciado adecuada validez interna y externa en predecir la mortalidad en el paciente séptico. Se realizó un estudio observacional de cohorte en 248 pacientes que ingresaron a la UCI con sepsis severa o choque séptico, para predecir la mortalidad a 28 días, sometiendo el puntaje PIRO a un proceso de validez externa. Se analizaron 248 pacientes a 28 días, con una mortalidad de 25.4%. El puntaje PIRO mostró un aumento progresivo en la mortalidad con el incremento del puntaje, con un área bajo la curva de 0.702 y una relación lineal entre la mortalidad esperada y la observada. En conclusión, este estudio ofrece a los paciente sépticos en estado crítico, un modelo predictivo de mortalidad con discriminación moderada y adecuada calibraciónes_CO
