dc.description.abstractAntecedente: La meta principal de paciente critico es mantener una adecuada DO2 a los tejidos, evitando la DEOx como indicador de DO2 crítico. El objetivo de este estudio es determinar la validez del DEOx a través de un cálculo cuantitativo indirecto para predecir FOM y mortalidad a 28 días en pacientes admitidos en UCI por infección por SARS-CoV-2 en relación al APACHE II, SOFA y 4C Score. Métodos: Estudio cohorte retrospectiva de pacientes con infección por SARS-CoV-2 admitidos en UCI entre el 2020 y 2021. Se calcularon al ingreso a UCI los scores de Acute Physiology and Chronic Health Evaluation (APACHE II), Sequential Organ Failure Assessment (SOFA) y 4C mortality comparando su precisión para predecir mortalidad a 28 días y FOM con el DEOx. Resultados: Se incluyeron 708 pacientes, con una tasa de mortalidad del 44,4% (n = 315), de los cuales 72,5% eran hombres con un valor de DEOx 11,16 mlO2/kg. La edad promedio fue de 58,7 (DE 14,51) años. El análisis multivariado se relaciona de manera independiente el DEOx para mortalidad, así como la intubación con OR de 8,69 (IC del 95%, 4.28 – 17.62) y lesión renal de 3,35 (IC del 95%, 2.40 – 4.68), y por cada punto de aumento de la creatinina el riesgo de FOM aumento con OR de 4.1 (IC del 95%, 1.87 – 8.98). Para determinar la precisión de las puntuaciones se realizó un análisis ACOR con una discriminación débil y comportamiento similar para los desenlaces primarios. La escala más precisa para mortalidad y FOM fueron el 4C con un AUC de 0.683 (IC del 95%, 0.64 – 0.72) y APACHE II con un AUC de 0,814 (IC del 95%, 0,77 - 0,85), mientras que el ACOR de DEOx fue 0,612 (IC del 95%, 0.57 – 0.65) y 0.646 (IC del 95%, 0.57 – 0.71) respectivamente. Conclusiones: La DEOx como predictor de FOM y mortalidad a 28 dias en paciente críticamente enfermo con SARS CoV 2 fue similar a los scores de APACHE II, SOFA y 4C.es_CO
