The true dichotomy between ethical problems and ethic aspects of pediatric practice lies on reductionism, which implies adopting dilemma as a problem prototype in medical practice. It is shown how common sense and good intentions are not only insufficient but also real problem generators. In contrast, weighed and careful reflection based on the purpose of the medical action allows us to make some decisions that are much better adjusted to the patients’ personal realities, to their dignity, and materialize in ethical action standards. Reflection on the ethical aspects of pediatric care focuses on three fields: primary care, pediatric surgery, and intensive care units (ICUs). In the first field, concepts such as the doctor-patient relationship, as well as sanitary care, decision-making, and the organization of pediatric assistance centers are reviewed. The second field is used to introduce a commentary making a distinction between two versions of Bioethics, i.e., the so-called principleism, and personalism. Finally, when referring to the ICUs, some outlines are offered, intended to provide an answer to major concerns arising from these specialized care units.; La real dicotomía entre problemas éticos y aspectos éticos de la práctica pediátrica está fundamentada en el reduccionismo, que implica adoptar el dilema como prototipo de problema en la práctica médica. Se muestra cómo el sentido común y las buenas intenciones son no sólo insuficientes, sino verdaderos generadores de problemas; en cambio, la reflexión ponderada y cuidadosa, apoyada en el objeto del acto médico, permite tomar determinaciones más ajustadas a la realidad personal del paciente, a su dignidad, que se concretan en pautas de acción éticas. La reflexión sobre los aspectos éticos de la atención pediátrica se centra en tres campos: la atención primaria, la cirugía pediátrica y las unidades de cuidado intensivo (UCI). En el primer campo se revisan conceptos como la relación médico-paciente, el tipo de atención sanitaria, la toma de decisiones y la organización de los centros de asistencia pediátrica. Se aprovecha el segundo campo para hacer un comentario que distingue dos versiones de la Bioética: el principialismo y el personalismo. Finalmente, al hablar de las UCI se ofrecen algunas pautas, que intentan responder a las principales inquietudes que surgen en esas unidades de atención especializada.

Suicide-risk management is an important component of medical practice especially in the assessment of patients with suicidal tendencies, the clinical treatment of whom should focus on them and their support network. The choice of a specific therapy depends not only on the judgment of the suicidal patient’s actual risk in psychiatric terms, but also on other patient-related issues. These dangerous behaviors can be lessened if we manage to understand suicide as a public health problem likely to be prevented, in which both individuals and groups can be involved. The principles of medical ethics, as applied to the assessment of this kind of patients, may serve both teachers and students as a decision-making guide and support.; El manejo del riesgo suicida constituye un componente importante de la práctica médica, especialmente en la evaluación de los pacientes, cuyo tratamiento clínico debe centrarse en ellos y en su red de apoyo. La elección de una terapia en particular depende no sólo de la estimación del riesgo suicida, desde el punto de vista psiquiátrico, sino también de otros aspectos relacionados con el paciente. Estos comportamientos pueden reducirse si se entiende el suicidio como un problema de salud pública, susceptible de prevención, en el cual se involucran tanto individuos como grupos. Los principios de ética médica aplicados a la evaluación de estos pacientes pueden servir de apoyo a maestros y estudiantes, para tomar decisiones.

A bioethical vision is exposed on the personal condition of the human embryo, based on its ontological reality. The human condition of the individual is affirmed as intrinsically worthy, noble, from the very beginning of its existence or biographical wandering as a zygote. Furthermore, in a critical way, a discussion is set out about some utilitarian-materialistic ideas used to deny the personal condition of the human embryo through the assignation of “extrinsic” and arbitrary criteria for dignity: age, development period, as well as the will of the mother or third parties, clinical, anatomic or physiological conditions, legislative decisions, and other kind of determinations. Likewise, attention is drawn on how in the current medical obstetric practice the ontological aspects of the individual condition of every human being as a person are being denied due to pressures of utilitarian nature. There is an “explicit” as well as a “tacit” negation of the personal reality of the unborn. It is affirmed that respect owed to the human being as a person since its very beginning - as deserved by any born/grown member of the human species - is due to reasons of “intrinsic” nature that are inherent to each particular individual condition, with no discrimination whatsoever, within the concept of the “teleological autonomy” of the human embryo.; Se presenta una visión bioética sobre la condición personal del embrión humano, con base en su realidad ontológica. Se afirma la condición humana de la persona, intrínsecamente digna desde el momento del inicio de su existencia o trayectoria biográfica como cigoto. Además, se comentan de modo crítico algunas ideas utilitaristas-materialistas, con las que se niega la condición personal del embrión humano mediante la asignación de criterios “extrínsecos” y arbitrarios de dignidad: edad, período de desarrollo, voluntad de la madre o de terceros, condiciones clínicas, anatómicas o fisiológicas, determinaciones legislativas y otras.Asimismo, se llama la atención sobre los modos como en la práctica médica obstétrica contemporánea se han venido negando los aspectos ontológicos de la condición personal de todo ser humano, debido a presiones de orden utilitario. Existen tanto la negación “explícita” como la negación “tácita” de la realidad personal del no nacido.Se afirma que el respeto debido a la persona humana desde sus inicios –como a todo miembro de la especie– se debe a razones de orden “intrínseco”, inherentes a la condición personal de cada quien, sin discriminación alguna, dentro del concepto de la “autonomía teleológica” del embrión humano.

The core axis of bioethical reflection is the personal molding of the physician as focused on the right approach of the doctor-patient relationship. That is why the crucial point of medical training is the relation between knowledge, understanding and communication. Where properly set out, it allows us to overcome the kind of dualism that counters scientific data objectivity with the subjectivity of life experiences of both the doctor and the patient. In order to grow in this disposition of mind in which on the one hand are involved the specific professional knowledge of the physicians and their human dimension of generosity and compassion on the other, a deep cultural education becomes necessary, where human sciences such as philosophy and literature contribute their knowledge of the homo patiens, which is undoubtedly indispensable to take care of the sick.; El eje de la reflexión bioética es la formación personal del médico, centrada en un correcto planteamiento de la relación médico-paciente. De aquí que el punto crucial de la formación médica sea la relación entre conocimiento, comprensión, interpretación y comunicación. Si está correctamente planteada, permite superar el dualismo que contrapone la objetividad de los datos científicos a la subjetividad de las vivencias, tanto del médico como del paciente. Para crecer en esta actitud, donde están implicados, por un lado, el conocimiento profesional específico del médico, y por otro, su dimensión humana de compartir con otro y de compasión, se hace necesaria una honda formación cultural, donde las ciencias humanas, como la filosofía y la literatura, aporten aquel conocimiento del homo patiens indispensable para hacerse cargo de su cuidado.
